A 90 year old man with a history or gradually deteriorating central vision self refers to your practice. The patient has noticed a decline in vision of the last few years, but this has become more noticeable in the last 3-4 months. Snellen visual acuity is 6/9, no improvement with pin hole, some distortion on Amsler charts, but the patient is a poor witness.
