The BMJ Case Reports journal documented how the girl had presented with mild right upper lid swelling for two weeks, before the symptoms worsened resulting in ‘bloodstained tears’.

She was then referred to an ophthalmologist as well as a radiologist for imaging. They found an oval-shaped posterior eyelid lesion (pictured) and an infective focus or epidermoid cyst was suspected in the radiology report.

‘This poses a challenge in making diagnosis because children may not be able to give a detailed history, and physical examination is more difficult,’ said ophthalmologist Dr Sek Keung Kwok.
