GOS forms update

From August 1 all GOS1 forms must be countersigned by a practitioner and contractor in accordance with changes in Department of Health legislation.

In circumstances where the optometrist is also the contractor or authorised signatory one signature is accepted.

New GOS forms were scheduled to be introduced from July 31, but as they will not be ready in time the DoH has said that in the meantime old GOS forms will need to be countersigned to comply with the regulations.

The changes will only affect optometrists and OMPs on supplementary lists.

To make a claim for money the contractor will need to sign the front of the form and the practitioner will sign the reverse. Practitioners must also include their list number.

The new forms are currently being distributed by the DoH to primary care trusts.
