What follows is the full determination as read out at by the GMC's fitness to practise panel.

'Dr Razzaq:

'This Panel has considered your case applying the General Medical Council’s Preliminary Proceedings Committee and Professional Conduct Committee (Procedure) Rules 1988. It has also had regard to the submissions of Counsel for both parties, and to the advice of the Legal Assessor.

'The Panel heard that from January 1999 you worked from time to time as an ophthalmic medical practitioner at Eagle Eyes Opticians in Wandsworth, South London. On 27 September 2000, Miss Heroda Berhane attended this opticians alone. She had just turned 18 and since childhood has been profoundly deaf. You have admitted and it has been found proved that you signed an NHS optical voucher certifying that you had tested her sight on that day. The Panel considered all of the oral and documentary evidence placed before it, including the NHS ‘GOS1’ and ‘GOS3’ forms containing your signatures.
