There's a West Country tale about a lost tourist which rather puts me in mind of the current situation the profession finds itself over payment for glaucoma primary care services.

A well-heeled tourist finding himself lost in a small village sees a yokel sitting by the side of the road. The tourist pulls up in his flashy car and asks the old boy how he would get to Exeter from here.

'If I wanted to go to Exeter, I wouldn't start from here,' he replies.

The frustration of the situation is infuriating. The tourist has the car, has a goal and has the money, the yokel knows the answer. The reply he gives to the tourist is the truth but provides little help. The implication is that he shouldn't set off without knowing where he is going and that he should have asked earlier.
