Optometrist Robert Frith, who runs a chain of practices in the South West, will face allegations that he did not perform a visual test on a patient and did not record the patient's family history of glaucoma.
He also faces an allegation of failing to complete adequate records. A hearing will begin in London on November 7.
Nathan Ryan, the Specsavers dispensing optician suspended in the summer for working under the influence of cocaine (News, June 16), faces a full hearing on November 9.
