Student DO struck off

A student dispensing optician who forged the signature of her pre-qualification supervisor on her final declaration forms has been erased from General Optical Council registers by a fitness to practise committee.

Hannah Montgomery had undertaken training at Specsavers in Shirley and was alleged to have forged the signature on forms dated December 31 2011.

The registrant, who did not attend the hearing, was said to have explained to the committee that she thought her supervisor’s signature was a confirmation of documents such as tracking sheets, which had already been signed.

The committee said it was a breach of the Code of Conduct which required registrants to be honest and trustworthy. In a written statement to the panel, Montgomery was said to have claimed she felt this was not the case, which the committee said showed a lack of insight into her actions.
